Items to bring to your Appointment

Please note that you will need to bring the following to your appointment: 

•  7 page packet completed in full – black or blue ink only please.

• Current insurance card.

• Valid driver’s license or photo identification card.

• Applicable co payment or consultation fee in cash, cashier’s check or money order.  We have an ATM machine with a $1.00 surcharge if this is more convenient for you. 

• If you have not met your deductible, please note that you will be required to pay it at the time of your visit.

• PLEASE NOTE THAT WE DO NOT ACCEPT CHECKS.

Please note that we will be forced to reschedule your appointment if you do not bring in the required items or are more than 15 minutes late for your scheduled appointment.
